Covid Testing
Carrier OTC COVID-19 Tests Aetna Members can submit a claim for reimbursement for tests purchased on or after January 15, 2022. Additional information will be available here once available Anthem Members can submit a reimbursement claim on member portal for tests purchased on or after January 15, 2022. Go to Claims & Payments, and choose Submit A Claim. Test can also be requested thru the Anthem COVID-19 Site. Click on “Find a Testing Center” – Enter your Zip Code – Scroll to “At-Home COVID-19 Test” section and click “Get Started” – Scroll down and client on the “Insurance Pay” button – Follow Prompts to Order Test.

Blue Shield Members can submit reimbursement claim for a test kit for tests purchased on or after January 15, 2022. Claim form and receipt will need to be submitted to Blue Shield. Click here for more information. Health Net Members can submit a reimbursement claim form for tests purchased on or after January 15, 2022 at www.healthnet.com or by calling the Customer Contact Center at the telephone number listed on their ID card. Kaiser Members can submit a reimbursement claim for tests purchased on or after January 15, 2022. You may be able to get a saliva home collection kit that you can send back to Kaiser Permanente for processing. If you’re eligible, you can choose to have a kit mailed to you during your e-visit. The kit should arrive at your address within 2 to 3 business days.
Sharp Members can submit claim reimbursement beginning January 15, 2022. UnitedHealthcare UnitedHealthcare’s initial preferred OTC retailer for at-home COVID-19 tests is Walmart Pharmacy, where members who have UnitedHealthcare Pharmacy benefits administered by OptumRx will have no up-front cost for tests purchased on or after January 15, 2022 and will not have to submit a form for reimbursement. UnitedHealthcare is working to add additional preferred retailers. If the member purchases an at-home COVID-19 test, on or after January 15, 2022, at any in-store or online retailer other than the in-store Walmart Pharmacy counter, they may submit purchase receipt(s) for reimbursement at the UnitedHealthcare member portal for a maximum reimbursement of $12 per test. Reimbursement for tests purchased at Walmart.com and the Walmart checkout counters found at the front of the store also must be submitted through the UnitedHealthcare member portal. Many COVID-19 tests are sold as a two-pack so that means the test pack would be reimbursed at $24 ($12 for each test).
